Student Organization Programming Proposal Form

Due to current COVID-19 guidelines, there are restrictions on who is permitted to host events on campus. If you wish to request an exception to this rule, please fill out this programming proposal document.

Forms are due no later than 3 business days prior to the event.

*Please note, submission of this document does not guarantee event approval. A meeting with administration may be required.

Have you included in your communication to attendees a reminder that if they have any COVID-19 symptoms, including any respiratory symptoms or are feeling unwell in any way, they should not attend the event? *
Does your event have hand washing stations and/or hand sanitizer stations? *
Will your event include routine cleaning before, during and after the event? *
Will your event include proper social distancing and maintaining 6 feet separation between attendees? *
Will your event require all attendees and staff to wear face coverings? *
Will your event serve food? *
Will your event include high contact activities (closer than 6 feet for greater than 15 minutes) for attendees? *
Current guidelines only permit TXST Faculty, Staff or Students to attend on campus events

Include your COVID mitigation strategies (Recommend only 2 individuals per row of vehicle seats)
Please provide a detailed plan of staff intended for this event
Please include set-up and tear-down information
